The unit must maintain medical records that permit determination of the degree and intensity of treatment provided to individuals who are furnished services in the unit; the unit must meet special staff requirements in that the unit must have adequate numbers of qualified professional and supportive staff to evaluate inpatients, formulate written, individualized, comprehensive treatment plans, provide active treatment measures and engage in discharge planning.

In general, a distinct unit within a general acute care hospital provides a comprehensive array of rehabilitation services for people with disabilities, along with all support services necessary to help patients attain their maximum functional capacity.

For Medicare, a distinct part of a general acute care hospital that provides inpatient rehabilitation services and meets the following requirements.

Rehabilitation Units have a pre-admission screening process in which the condition and medical history of each prospective patient are reviewed. This assessment determines whether the patient is likely to benefit significantly from an intensive inpatient program.

The Rehabilitation Units ensure that patients receive close medical supervision and that rehabilitation services are provided by qualified personnel. These services include nursing care, physical therapy, occupational therapy, and, as needed, speech therapy, social services, psychological services, and orthotic and prosthetic services.

Each inpatient has an individualized treatment plan established, reviewed, and revised as necessary by a physician in consultation with other professional staff involved in the patient’s care.

A coordinated multidisciplinary team approach is used to rehabilitate each inpatient, with periodic clinical entries documented in the patient’s medical record to track progress toward treatment goals. Team conferences are held at least every two weeks to assess the appropriateness of the treatment plan.

The unit is overseen by a director of rehabilitation who provides services for at least 20 hours per week.

This director must be a doctor of medicine or osteopathy, licensed under state law to practice medicine or surgery, and must have completed at least two years of training or experience in managing inpatients requiring rehabilitation services, following a one-year hospital internship.nship.ip.
