Case Management Registered Nurses (RNs), also known as nurse case managers, coordinate and manage patient care to ensure that patients receive appropriate, efficient, and effective treatment, particularly for complex or chronic conditions.

They create and implement individualized care plans, acting as liaisons and patient advocates between patients, their families, and healthcare providers.

Key duties include assessing patient needs, educating patients and families, coordinating various care services, facilitating communication among the healthcare team, and advocating for the patient’s well-being.
