Arizona Exclusion Screening Requirements
The Arizona Health Care Cost Containment System (AHCCCS) is responsible for identifying, preventing, and prosecuting fraud, abuse, and misuse in the public healthcare programs.
To ensure existing employees or contractors, and new hires have not been excluded from participation in the Medicaid program, providers participating or applying to participate in the Medicaid program must search the following exclusion lists monthly:

- Arizona State Exclusion List
- Health & Human Services (HHS) Office of Inspector General (OIG) List of Excluded Individuals and Entities (LEIE)
- General Services Administration (GSA) System for Award Management (SAM)
